Job Description
The Care Coordinator plays a crucial role in providing intake, phone support, referrals, and clerical services within a Referral and Treatment Call Center. This role offers the opportunity to help individuals in Hawai'i seeking substance use treatment and mental health services, by delivering timely and compassionate assistance. As a Care Coordinator, you will provide vital support for those in need, ensuring they receive the guidance and services they require.
Key Responsibilities:
Respond to incoming inquiries through phone calls, text messages, emails, and live web chats, offering information, screening, and support for individuals seeking help with substance use treatment, recovery services, and mental health needs.
Complete client intake profiles and screening processes to determine the appropriate support.
Offer compassionate counseling and conduct crisis assessments, assisting clients in accessing the services they need.
Utilize motivational interviewing and trauma-informed care principles to provide empathetic support and effective solutions.
Ensure all necessary agency and program requirements are met during the referral process.
Review and confirm pre-treatment services for clients who self-refer.
Collaborate with senior care coordinators and clinical supervisors to ensure ongoing quality assurance and client follow-up.
Conduct clinical reviews and assessments based on each client's unique behavioral health needs.
Stay informed about program procedures and service requirements to ensure high-quality, accurate assistance.
Additional Duties:
Provide clear, accurate, and timely information and referrals for substance use disorder (SUD) and mental health services.
Perform data entry for updates and new resources.
Stay knowledgeable about safety protocols and emergency response procedures within the program.
Assist with other duties as needed to support the team and ensure smooth operations.
Qualifications:
High school diploma or GED required; coursework in social work, human services, public health, or a related field is preferred.
At least 1 year of experience in direct service work within social services, human services, or nonprofit organizations (experience in substance use/mental health is not required).
Previous experience in client services, particularly in nonprofit or social service settings, is highly preferred.
Proficiency with computer applications, including Microsoft Office and database management software.
