About the Role:
As a Community Partner Care Coordinator, you will play a key role in helping individuals navigate their healthcare journey, ensuring they receive the care they need at the right time. You'll work closely with members, building strong, trusting relationships to support them in accessing both medical and social resources. Your efforts will focus on assisting members in understanding and utilizing their Medicaid, Medicare Advantage, and/or Dual Eligible Special Needs Plan (DSNP) benefits, while also addressing their social determinants of health (SDOH). You will be an advocate, helping to empower members to manage their healthcare and make informed decisions about their well-being.
What You'll Do:
Conduct Outreach: Reach out to members to educate and encourage participation in healthcare programs and services.
Assess and Develop Care Plans: Collaborate with members to assess their health status and create tailored care plans that align with their personal goals. This is done primarily via phone and text.
Coordinate Care: Work with healthcare providers and other partners to ensure members receive timely care, including scheduling appointments and following up as necessary.
Support Social Needs: Identify and address needs related to Social Determinants of Health, ensuring holistic support for each member's well-being.
Document and Communicate: Accurately document interactions and actions taken in the care management system, and effectively communicate with members, families, providers, and other stakeholders.
Advocate for Members: Be a trusted voice, providing guidance and motivation to help members manage their health and healthcare with confidence.
What You'll Need:
Experience: At least 2 years of experience in roles such as care coordination, case management, community health work, social work, or pharmacy technology.
Education/Certification: LPN or RN preferred, though not required.
Language Skills: Proficiency in English is required; fluency in Spanish is a plus.
Location: Must be based in Tennessee.
Technical Skills: Comfortable using Apple computers (e.g., MacBook), documenting case notes in care management or electronic health records (EHR) platforms.
Experience with Medicaid/Medicare: Previous experience working with Medicaid, Medicare Advantage, and/or Dual Eligible patients is a plus.
Required Skills:
Strong interpersonal and communication skills, both verbal and written.
Effective problem-solving abilities.
Experience working with customers over the phone and through text messages.
Familiarity with motivational interviewing techniques.

Full-time, 8-hour shifts, Monday to Friday (during regular business hours).
